Populations Outcome Management

Engaging program designed to assist members in the home with Acute and Chronic Respiratory Illness, such as Chronic Obstructive Pulmonary Disease (COPD), Obstructive Sleep Apnea (OSA) and Asthma, with the goal of improved member understanding of their disease process and compliance with their respiratory regimen.

Our POM program utilizes the clinical knowledge of liaisons to provide a comprehensive approach to achieve member focused care. The individualized approach provides members with the tools to promote self-management.

Members are provided with a Respiratory Therapist 24/7 that has resulted in a decrease of utilization management (62% reduction in hospital occurrence) and improved quality of life.
